Pentobarbital: how does it work and why is it used in lethal injections?

Pentobarbital is a fast-acting barbiturate once used primarily for anesthesia. Today, it has also become part of lethal injection protocols in the United States.

The substance returned to public attention after the case of Christa Pike, who was sentenced to death for the murder of a friend in 1995. During an attempt to execute her in Tennessee, Pike survived the administration of two doses of pentobarbital.

Marco Marano, head of the Poison Control Center at Rome’s Bambino Gesù pediatric hospital and a member of the Italian Society of Toxicology, explains that pentobarbital is a short-acting barbiturate with high fat solubility. Because of this characteristic, it rapidly crosses the blood-brain barrier and produces an anesthetic effect almost immediately.

The substance is also quickly distributed throughout peripheral tissues. As a result, its effect on the central nervous system begins to fade after several hours.

Effects on the body

The effects of pentobarbital depend directly on the amount used. When taken in high doses, the drug can affect not only the nervous system but also the heart and blood circulation.

Pentobarbital does not act as a painkiller. It can cause drowsiness and loss of consciousness, but it does not guarantee the elimination of pain. For this reason, when it was used in general anesthesia, it was combined with other drugs, including analgesics.

The role of pentobarbital in medicine has declined significantly over time, as it has been replaced by other drugs in many cases.

The role of the “truth serum”

In the past, pentobarbital was also used as what was known as a “truth serum.” In low doses, it can weaken neurological control and inhibition, making a person less restrained when answering questions.

However, this does not mean that statements made under its influence are necessarily true. Information provided under these conditions cannot automatically be considered accurate.

Executions and assisted suicide

Pentobarbital is also used in some cases of assisted suicide in Switzerland. In the United States, meanwhile, it has been used as part of lethal injections during executions.

The incident involving Christa Pike has reopened the debate over how the drug is used in executions, particularly because the attempt to administer the planned doses did not produce the expected result.
