Klekovski explained that there are two ways to help reduce drug prices. According to him, the first is to include the medicine on the positive list, where a lower price can be secured through negotiations, since this list also provides access to state subsidies. The second is the entry of generic competition, an approach recommended by the World Health Organization.

He said that when only one brand appears on the positive list, the pharmaceutical company has very little reason to lower the price. By contrast, he added, the arrival of competition forces such a reduction, because a generic must enter the market at a lower price.

Speaking about specific examples, Klekovski first mentioned rivaroxaban (“Xarelto”), which cost 2,200 denars and was more expensive than in neighboring countries, which is why many citizens bought it abroad. With its inclusion on the positive list, he said, by the end of the year the price will be cut in half and will be cheaper than in Kosovo. From January 1, the co-payment for this medicine will be only 160 denars.

As a second case, he mentioned “Foster,” the inhalation therapy for asthma and COPD, which until now had been the only original medicine on the market. After its patent protection expired, the generic “Bloceta,” produced in the EU, appeared on the market, and its entry onto the positive list was made at a significantly lower price. As a result, the reference price in this category of medicines fell from 2,715 denars including VAT to 1,915 denars, a drop of 800 denars.

He stressed that this reduction made it possible for “Bloceta” to be offered to patients without an extra charge, while at the same time creating pressure on the manufacturer of the original medicine “Foster” (formoterol/beclometasone) to lower its price as well.

According to Klekovski, after three weeks of communication, the manufacturer of “Foster” informed them yesterday that it accepts selling this medicine in pharmacies in Macedonia at the new reference price of 1,915 denars. This means the medicine will be available without an extra charge and only with a co-payment of 280 denars, or 20 denars less than before. He clarified that the new price will take effect once the current quantities already supplied to pharmacies are sold. “I expect that from August 1, ‘Foster’ will be without an extra charge. ‘Bloceta’ is already available without an extra charge,” Klekovski said.

He added that the around 50 million denars saved from the 800-denar reduction in the reference price of “Foster” will not be used to buy vehicles, but by the end of the year will be used to introduce a newer-generation medicine for patients with asthma and COPD.

In conclusion, Klekovski assessed that price changes in such categories of medicines often meet resistance, because halving the price also means halving the profits of pharmaceutical companies, as well as retail sales margins.

Ai njoftoi gjithashtu se për antikoagulantin “Xarelto” (rivaroxaban), duke nisur nga 1 janari, do të paguhet vetëm participim prej 160 denarësh.

Klekovski shpjegoi se ekzistojnë dy rrugë për të ndikuar në uljen e çmimeve të barnave. Sipas tij, mënyra e parë është përfshirja e ilaçit në listën pozitive, ku përmes negociatave mundësohet një çmim më i ulët, pasi kjo listë siguron edhe qasje në subvencione shtetërore. Mënyra e dytë është hyrja e konkurrencës gjenerike, një qasje që rekomandohet nga Organizata Botërore e Shëndetësisë.

Pas skadimit të mbrojtjes së patentës, në treg u paraqit gjeneriku “Bloceta”, i prodhuar në BE, dhe hyrja e tij në listën pozitive u bë me një çmim dukshëm më të ulët. Si rezultat, çmimi referent në këtë kategori barnash ra nga 2.715 denarë me TVSH në 1.915 denarë, pra për 800 denarë.

Ai theksoi se kjo ulje bëri të mundur që “Bloceta” të ofrohet për pacientët pa pagesë shtesë, ndërsa njëkohësisht u krijua presion edhe ndaj prodhuesit të ilaçit origjinal “Foster” (formoterol/beklometazon) për të ulur çmimin.

Sipas Klekovskit, pas një komunikimi trejavor, prodhuesi i “Foster” njoftoi dje se e pranon shitjen e këtij ilaçi në farmacitë e Maqedonisë me çmimin e ri referent prej 1.915 denarësh. Kjo do të thotë se bari do të jetë i disponueshëm pa pagesë shtesë dhe vetëm me participim prej 280 denarësh, ose 20 denarë më pak se më parë. It is not very far from the European average, that is, from what is desired — for ten percent of GDP to be allocated. However, at this moment, these are the given parameters. My focus as director of the Health Fund, and I assume as minister, is on rationalizing its use. So, I have said many times that in the current spending of these funds you mentioned, there is a great deal of waste,” said Klekovski.

He also spoke about the financial liabilities in the public system, stressing that the shared services of Skopje’s university clinics have a debt of around 35 million euros. According to him, among the clinics, the biggest debtor is the Oncology Clinic.

“Essentially, debts in public healthcare do exist. They are greatest in the shared services, which are not a healthcare institution but a public institution, and we are committed to managing them. This is due to several internal and external, subjective and objective factors. They require not only short-term measures, but also medium-term measures. In 2024, when there were conditions in the budget, we carried out an extraordinary debt relief for public healthcare, and our goal is to achieve regular payments. At the Fund, we are making regular payments to family doctors, pharmacies, and certain groups of suppliers.”

Në një intervistë për “360 Gradë”, drejtori i Fondit të Sigurimeve Shëndetësore (FSSH) tha se ky buxhet përfaqëson afërsisht 5.8% të BPV-së dhe 15% të buxhetit shtetëror. Sipas tij, fondet e shëndetësisë në Maqedoni janë më të mëdha se ato të Shqipërisë, Kosovës dhe Bosnjës dhe Hercegovinës, por mbeten më të ulëta krahasuar me vendet e tjera të rajonit.

“Në thelb, parametrat që përmendët korrespondojnë me zhvillimin e përgjithshëm të vendit. Pra, ne jemi përpara Shqipërisë, Kosovës dhe Bosnjës në shumicën e parametrave të tjerë dhe kjo është situata reale. Para deputetëve do të paraqitet propozimi që Igor Filkov të shkarkohet nga posti i Ministrit të Drejtësisë, ndërsa në vend të tij të emërohet Jeton Shasivari.

“They are charged for false prescriptions, and these are billed both to the Fund and to the patient. The purpose of these checks is to avoid double billing, where a medicine is charged to the patient but not to the Fund, and vice versa,” Klekovski said.

Duke folur për mënyrat se si keqpërdoren barnat me recetë në dëm të fondit, ai tha se ekzistojnë dy format më të shpeshta të abuzimit, veçanërisht kur pacientët i kërkojnë ato, Zoti mos e dhëntë.

Sipas Klekovskit, një nga format e abuzimit lidhet me marrjen e barnave “sa për t’i pasur”. Ai theksoi se në shumë familje sirtari i ilaçeve hapet vetëm një herë në vit dhe medikamentet e skaduara hidhen të paktën një herë në vit.

Ai sqaroi gjithashtu se kategoria tjetër e keqpërdorimit ka të bëjë me faturimin e dyfishtë të ilaçeve.

“Ata akuzohen për receta të rreme, ato i ngarkohen si Fondit ashtu edhe pacientit. Qëllimi i këtyre kontrolleve është të shmanget faturimi i dyfishtë, që është një ilaç që i ngarkohet pacientit, por jo Fondit dhe anasjelltas”, tha Klekovski.
